If you are discussion about reducing uric bitter in proclaim to treat gout, here is a good discussion, though possibly a bit too scientific. The short altered copy is that the two main drugs are Allopurinol and Probencid. if you want the together discussion, read on:
Two choices are available to lower serum uric acid level. Probencid can be given to patients with decrease clearance of uric acid from the kidney. Patients must own documented decrease within urine uric acid and hold a creatinine clearance >40 cc/min. Allopurinol can be used in patients who any have decrease clearance of uric acid or elevated synthesis of uric acerbic.
Allopurinol
* xanthine oxidase inhibitor
* prevents production of uric acid
* adjectives in both patients next to increased synthesis and decreased clearance of uric bitter
* no 24 hour urine needed
* can be used in renal washout
* rarely associated near bone marrow suppression, hepatotoxicity, and hypersensitivity reactions
Although 90% of patients near hyperuricemia have decrease clearance of uric acid, lots have poor renal function. Patients are habitually poorly compliant with requests for 24 hour urine sample. Thus, my and the majority of physicians' approach is to use allopurinol in most cases. Allopurinol is started at 100 mg day after day (100 mg qod if creatinine clearance < 10 cc/min) and is titrated by 100 mg every 10–14 days to achieve a serum uric bitter level of 4–5 mg/dl. The maximum dose of allopurinol is 800 mg day after day. Liver transaminases, complete blood counts and renal function and should be monitored. Toxicites include rash, hepatoxicity, bone marrow suppression and severe hypersensitivity reaction. There are desensitization protocols for allopurinol allergic patients.(ref 9) Allopurinol can raise the level of warfarin and theophylline and levels should be monitored. Its use within patients on azathiprine, 6–mercaptopurine and cyclophosphamide should be either avoided or used near intensive monitoring for bone marrow toxicity. Initiation of allopurinol can prolong an acute attack of gout if started during the attack, or trigger a gout attack due to shifts in uric tart levels. It is adjectives to give prophylactic colchicine when starting allopurinol and tip off the patient in the region of the possibility of a flare.
Probenecid
* uricosuric
* decreases uric bitter reabsorption at the proximal renal tubules
* useful contained by patients with decrease renal clearance of uric acid
* can single be used if creatinine clearance >40 cc/min
* must have 24 hour urine for uric bitter <800 mg/dl
* can be used in renal fiasco
* increased risk of renal stones
Probenecid may be given to patients with decrease clearance of uric acid by the kidney and usual renal function. In general its use should be controlled to patients under the age of 60. Probenecid act by inhibiting reabsorption of uric acid surrounded by the proximal tubules of the kidney. Starting dose is at 500 mg to 1000 mg daily and increased to 1500 mg to 2000 mg as needed. Occasionally superior doses are needed. Probenecid may precipitate renal stone formation and good oral hydration should be prompted. Probenecid is contraindicated in patients beside renal stones (including calcium and uric acid stones) and contained by patients with urate nephropathy. Probenecid given inappropriately to patients beside hyperuricemia due to overproduction of uric acid can motivation renal stones and urate nephropathy.
Good luck!!
